Enrollment Information for 2026–2027 School Year
Dear Parents/Guardians,
Please take a moment and complete the attached pre-enrollment packet.
All forms must be completed in their entirety, and each form must include your child’s name. Bethel Public Schools will also collect shot records and proof of residency documentation. Residency documentation must be a utility bill or a renters' agreement. Please note that all students are required to have completed pre-enrollment documents and proof of residency documentation on file in order to maintain enrollment.
Please remember that no medications will be administered without written parental consent, including Tylenol or other pain relievers. All medications must be provided directly to the school office by a parent or guardian. In order for the office to administer any medication, parents must complete and sign the board-approved “Medical/Medicine Form – Parent Authorization to Administer.”
It is imperative that you notify the school office immediately of any address or phone number changes throughout the year. If you move, you must provide the school office with current proof of residency, such as a utility bill in the parent or guardian’s name.
